Missouri nondriver template

This is a template for the old (a new one came out in 2004) nondriver
template. It will still be valid through 2010, though. I'm new at
making templates, so feel free to change it however you want.

I removed my post with the link. I forgot to take out my friend's
original license I used as a base (I'm a idiot). If you have the
template and want to share it, I beg of you to either remove the
picture of the license or blur out the information on it. I made the
template for you, so you can return the favor by doing that.
